Planned in 3D Before Surgery
Every implant case is planned on a 3D CBCT (cone-beam CT) scan, which shows the bone, the nerve canal and the sinus before the first incision. Dr. Fatima places single and multiple implants here; for All-on-4 dental implants she plans the case and makes your final teeth in collaboration with a trusted local oral surgeon, who places the implants.
Planned Around the Final Tooth
The implant’s angle and depth are planned on your 3D CBCT scan, around where the final tooth needs to sit. When your case calls for it, a surgical guide made from that plan is used during placement.

Chew With Confidence Again
Why Implants, Not a Bridge or Denture
Implants are the only option that replaces the root. In a large review, about 96% of implants were still in place after 10 years (Howe et al., J Dent 2019); the crown on top may need replacing over time.
Biocompatible Titanium
Medical-grade titanium integrates with your jawbone to create a stable foundation that your body can accept naturally.
Bone Preservation
Unlike bridges or dentures, implants stimulate the jawbone and help preserve bone after tooth extraction.
Lifelike Aesthetics
Custom-crafted porcelain crowns are shade-matched to your surrounding teeth for a natural-looking blend.
Long-Term Durability
With proper care and regular dental exams, dental implants can serve as a long-term tooth replacement in many well-maintained cases.
One Dentist Plans, Places and Restores Your Implant
There is the titanium root a surgeon puts in the bone, and there is the tooth that gets built on top of it. Often those are two offices, two records, two people who each did their part correctly. When the bite is a fraction off, or the crown traps plaque, or the shade never quite matches, there is nobody whose job it was to own the whole thing.
Most implant problems show up around the gum and the crown (an infection, or a loose or chipped crown) far more often than an implant fails to fuse. That is why the person placing it should plan it around the final tooth.
Dr. Fatima is credentialed on both sides. Her fellowship with the International Congress of Oral Implantologists and her surgical training at the Live Implant Training Institute cover the surgical half. Her fellowship with the American Academy of Implant Prosthodontics is the restorative half — the bite, the shape, the way the finished tooth cleans. Two implant fellowships and the surgical training certificate, all of them framed on the office wall where you can read them.
So she plans your case around where the final tooth needs to sit, places the implant here in this office, planned on 3D CBCT imaging, and makes the tooth that goes on it. One person, one plan, from the first scan to the day you chew with it. On a full-arch All-on-4, Dr. Fatima works in collaboration with a trusted local oral surgeon, who places the four implants; she does everything either side of that — still one treatment plan, still one person answerable for the result.
What that means on the day
- No hand-off, no re-explaining. You are not repeating your history to a second office that has never met you.
- One estimate, in writing, before anything starts. Not a surgical fee here and a restorative fee you find out about in four months.
- If something needs adjusting, it is adjusted here. By the person who planned it.

Choosing the Right Implant Provider
Not all implant practices deliver the same level of care. Before committing, ask any provider:
- Is the quoted fee all-inclusive, or are imaging, abutment, and crown billed separately?
- Does the dentist placing my implant have advanced fellowship training, or is implant placement a recent addition to their general practice?
- What implant system do you use, and does it carry a manufacturer warranty?
- Are all follow-up visits included in the treatment fee?
These questions help you compare true value, not just the initial quote.
Many patients choose to finance their treatment through CareCredit or Cherry, with monthly payments; terms depend on the lender and your approved offer. No insurance? Our in-house membership plan includes exams, cleanings, and treatment discounts.
Actual payment varies by treatment plan and credit approval. A personalized estimate is provided at your complimentary consultation.

What the Research Says
Published studies on this treatment, each summarized from the paper’s own abstract and linked to its PubMed record, including where the evidence is limited.
About 96% of implants of current design were still in place after 10 years (96.4%); the risk of losing an implant was higher in patients 65 and older
Implants carrying a single crown: 97.2% still in place at 5 years and 95.2% at 10 years; the crowns themselves 96.3% and 89.4%
After an extraction the jaw ridge loses 29–63% of its width and 11–22% of its height within about six months
Peri-implantitis (infection with bone loss around an implant) had an average prevalence of 22% across studies (range 1–47%), which is why implants need home care and professional cleanings

Infection Around the Implant
Peri-implantitis (infection with bone loss around an implant) affects about 1 in 5 implant patients over time in studies (22%; range 1–47%). Daily home care, regular cleanings and not smoking help lower the risk. We monitor healing at every follow-up visit and prescribe antibiotics when they are indicated.
Implant Failure / Non-Integration
About 96% of implants are still in place 10 years after placement; a small share do not integrate or are lost later. Risk factors include smoking, uncontrolled diabetes, and insufficient bone density. We use 3D CBCT imaging to assess bone volume before placement and discuss all risk factors openly at your consultation. When a bone graft is needed first, it can heal slowly or fail to take, which delays placement; we check how it has healed before placing the implant.
Temporary Nerve Sensitivity
After lower-jaw implant surgery, about 13% of patients notice numbness or tingling in the lip, chin or gums at 10 days, and about 3% at one year; about 9 in 10 of those affected recover within a year. We measure the distance to the nerve on your 3D CBCT scan before placement.
Crown and Screw Problems
Chipped porcelain is the most common later problem with implant bridges, and screws can loosen. In a 5-year review, about a third of implant-bridge patients had some complication. We look for both at your regular check-ups.
The jawbone starts to shrink as soon as a tooth is removed, most of it in the first few months: in studies the ridge lost 29–63% of its width within about six months (Tan et al., 2012). Placing an implant sooner can reduce the chance you need a graft.

Typical ranges for time, visits, and recovery from our routine cases.
- How many visits does it take?
- Several visits over the full treatment arc: consultation with 3D CBCT, surgical placement, healing check-ins, and final crown placement. Dr. Fatima tells you how many your case needs at the consultation.
- How long does each implant procedure take?
- Single-implant surgical placement is typically 60–90 minutes per appointment with local anesthesia.
- What's the healing timeline?
- Osseointegration (the implant fusing with the jawbone) takes 3–6 months before the custom crown is placed.
- How long do implants last?
- In a large review, about 96% of implants were still in place 10 years after placement (Howe et al., J Dent 2019). Daily cleaning and regular checkups help them last; the crown on top may need replacing over time.
Times vary by case complexity. These are typical ranges for the procedure; Dr. Fatima confirms your own timeline at your consultation.
Dental Implant FAQs
Dr. Fatima places and restores implants here, in this office, using 3D CBCT planning — single implants, multiple implants, and the crowns or bridges that go on them. She is a general dentist with implant fellowships from the International Congress of Oral Implantologists and the American Academy of Implant Prosthodontics, and surgical implant training from the Live Implant Training Institute. The one exception is All-on-4 full-arch treatment: a local oral surgeon performs the surgical placement of those four implants, and Dr. Fatima plans the case and makes and fits the final teeth, under one treatment plan.
The implant is placed under local anesthetic to numb the area, so most patients feel pressure rather than pain during the procedure; if you do feel discomfort, Dr. Fatima can add more anesthetic. Afterward, most patients have mild-to-moderate soreness for a few days that over-the-counter pain relievers typically manage well, and many find it more comfortable than they expected — often comparable to a tooth extraction. For anxious patients, Dr. Fatima uses topical numbing to help keep the injection comfortable.
Many healthy adults missing one or more teeth are candidates for dental implants. You need healthy gums, adequate jawbone density, and a commitment to meticulous oral hygiene. Conditions like uncontrolled diabetes, active periodontal disease, heavy smoking, or significant bone loss may require treatment or a modified plan before implant placement. Our evaluation process includes a 3D CBCT scan to measure your bone volume and a thorough medical history review. Call (508) 481-0110 to schedule a complimentary implant consultation with Dr. Fatima, our fellowship-trained implant dentist (FICOI, FAAIP).
Innova Smiles in Marlborough, MA plans and restores dental implants, including single implants, implant bridges, All-on-4 full-arch and implant-supported dentures. Dr. Ambereen Fatima holds two implant fellowships (FICOI, FAAIP), plans every implant case on 3D CBCT imaging and places single and multiple implants here; for All-on-4, a trusted local oral surgeon places the implants and she plans the case and makes the final teeth. When a case benefits from additional expertise, she coordinates referral or co-management with the appropriate specialist. Patients come from across MetroWest. Your fee is quoted in writing at the consultation, your benefits are checked once you book and return your forms, and we offer 0% APR financing through Cherry, subject to credit approval, and we also accept CareCredit.
The entire implant process typically spans several months. It begins with a comprehensive consultation including 3D CBCT imaging to plan your implant placement. After safely placing the titanium post into your jaw, we allow 3 to 6 months for the bone to integrate around it, a process called osseointegration. During this healing period, you may wear a temporary restoration so you are not without a tooth. Once healing is confirmed, we attach a custom porcelain crown that is color-matched to your natural teeth and designed to restore function and appearance. Many patients are pleasantly surprised by how smooth and manageable each phase is. Book your free implant consultation at (508) 481-0110 to get a personalized treatment timeline.
The cost of a dental implant depends on your specific case, including whether a bone graft is needed, the type of restoration (single crown, bridge, or full arch), and the overall complexity. Because no two cases are alike, we give you an exact, itemized written estimate at your free consultation rather than a one-size-fits-all number. Many PPO plans pay part of the implant crown or the implant, often after a waiting period and up to your annual maximum; some exclude implants. We check your plan once you book and return your forms, and give you a written estimate before treatment. For anything insurance does not cover, we offer 0% APR financing through Cherry, subject to credit approval, and our In-House Membership Plan members receive a discount on the fee. Call (508) 481-0110 to schedule your consultation.
With excellent oral hygiene, a dental implant can easily last for several decades or more. Caring for them is virtually identical to caring for your natural teeth. You must brush twice daily, meticulously floss around the implant crown, and attend your regular six-month checkups and professional cleanings.
Dental implants are one of the most predictable procedures in dentistry, with long-term success rates widely reported in the high 90 percent range for healthy patients. Success depends on good bone support, healthy gums, not smoking, and regular hygiene and checkups. The titanium post is designed to last for decades — sometimes far longer with good care — while the crown attached to it may need replacement over the years, much like a natural-tooth crown. At Innova Smiles in Marlborough, MA, Dr. Ambereen Fatima plans each case with CBCT 3D imaging to support a strong, lasting result.
Implant failure is uncommon, but it can happen — most often from infection around the implant (peri-implantitis), inadequate bone support, or heavy grinding. Warning signs to watch for include increasing pain or tenderness, a loose or shifting implant, swelling or redness of the gum around it, gum recession that exposes the implant, or difficulty chewing on that side. Early problems are often manageable when caught quickly, so call us promptly if you notice any of these. Seek urgent care for severe pain, swelling, or fever.
A dental implant replaces a single missing tooth without altering the neighboring teeth and helps preserve the jawbone; it is fixed and cared for like a natural tooth. A bridge replaces one or a few missing teeth by crowning the adjacent teeth, and is usually faster and lower in upfront cost. Dentures replace many or all teeth and are removable, or can be secured with implants for added stability. Implants typically offer the best long-term value and bone preservation, while bridges and dentures can be a good fit for specific situations and budgets. Dr. Fatima will review the trade-offs with you at a consultation.
True biological rejection of a dental implant is extremely rare because they are made of biocompatible titanium or zirconia, which the body naturally accepts. Dental implants have a documented success rate of 95 to 98 percent. Most so-called 'failures' are actually due to issues like poor oral hygiene, smoking, or uncontrolled medical conditions such as unmanaged diabetes that affect bone healing. Warning signs of an issue include persistent pain, swelling, or mobility of the implant post. Our fellowship-trained implant dentist carefully screens every patient with 3D CBCT imaging and a thorough medical history review to minimize any risk factors before placement. Call (508) 481-0110 to schedule your implant evaluation.
The timing depends on your bone health and the reason for the extraction. In some cases, we can perform an 'immediate implant' on the same day the tooth is removed. More commonly, we wait 10–12 weeks for the extraction site to heal and the bone to stabilize.
Starting your dental implant process involves a consultation with 3D CBCT imaging and a RAYFace face scan. Dr. Ambereen Fatima uses this data to digitally plan your implant placement so the final tooth looks natural and functions well. Book a free implant consultation online today.
