What this plan is, in plain terms
- It is not insurance. The Innova Smiles membership is a discount savings plan, not dental insurance, and it is not a substitute for dental insurance.
- The discount is a flat 20%. Members pay 20% less than our standard fees on any treatment that is not already included in their plan. It is the same 20% on every procedure — there are no tiers and no annual cap on the savings.
- We run it ourselves. The plan is offered directly by Innova Smiles. No insurer or other third party pays for, reimburses, or administers any care you receive under it.
- You pay at the time of service. Treatment is paid for when it is provided, less your membership savings. The annual fee is separate and is paid in full at enrollment.

The following Terms and Conditions describe the Innova Smiles In-House Membership Program. This information is provided for patient reference. Enrollment and payments are handled in-office; no online signature is required.
- This is NOT Insurance. I understand that this is a private discount membership program offered exclusively by Innova Smiles and is NOT a dental insurance plan. It is not a qualified health plan under the Affordable Care Act.
- Annual Fee is Non-Refundable. The annual membership fee is non-refundable, and no refunds will be given for any reason, including failure to use included services.
- No Rollovers. All services included in the membership (such as cleanings, exams, and x-rays) must be used within the 12-month membership period. Unused services will NOT roll over into the following year and have no cash value.
- Practice-Specific Program. This membership is valid ONLY for services performed at Innova Smiles by Dr. Ambereen Fatima. It does NOT include any services performed by specialists (such as oral surgeons, endodontists, periodontists, or orthodontists) to whom I may be referred.
- Exclusions. The 20% discount is a flat 20% off our standard fees for any dental procedure completed at our office that is not already included in the plan. It does not include retail products (such as electric toothbrushes).
- Combining Discounts. The annual membership fee is paid in full at enrollment. Membership savings cannot be combined with dental insurance benefits. They CAN be combined with any Innova Smiles promotion we identify as membership-eligible; for all other promotions, if more than one discount could apply, we apply whichever saves you more. This program is intended for patients without dental insurance. Members are welcome to use third-party financing for the cost of treatment.
- Clinical Necessity. The specific dental services provided (including the type of cleaning) are determined by the clinical diagnosis and recommendation of the dentist. This plan requires patients with periodontal (gum) disease to enroll in the Gum Health Membership to receive the clinically appropriate care (periodontal maintenance).
- Payment at Time of Service. The annual membership fee must be paid in full to activate the membership. All payments for services (including the 20% discounted portion) are due at the time of treatment.
- Renewals. Membership will not automatically renew. Innova Smiles will notify me before my membership expires with the option to renew at the terms and fees then in effect.
