Taking the step to book a first dental appointment--especially if it has been a while--is something we genuinely celebrate. New to Marlborough, switching dentists, or returning after a long gap? This new patient dental visit guide covers everything you need to make your first time at the dentist smooth, comfortable, and productive.
The ADA recommends dental visits at least once every six months for most adults, yet the CDC reports that roughly 35 percent of American adults did not visit a dentist in the past year. If you are in that group, you are not alone, and there is zero judgment here. Our entire first-visit process is designed around the understanding that many patients are walking through our doors after years away from a dental chair.
Before Your Visit: What to Bring to Your Dental Appointment
To make the most of your first appointment, please bring:
- Valid photo ID (driver's license, state ID, or passport)
- Dental insurance card(s) — if you have coverage, we will verify your benefits and explain exactly what is covered before any treatment begins
- Medical insurance card — some dental procedures (TMJ treatment, oral surgery, sleep apnea appliances) may be billed to medical insurance
- List of current medications — including supplements, over-the-counter medications, and any blood thinners or bisphosphonates (these affect treatment planning)
- Any recent dental X-rays — if you have X-rays from a previous dentist (even on a CD, USB, or emailed from your prior office), bring them so we can avoid duplicating imaging and save you time and cost
- Your completed new patient forms — you can fill these out online before your visit to save 10 to 15 minutes in the office
- A list of questions or concerns — write down anything you want to ask. Common questions include: "Why does this tooth hurt when I chew?", "Are my gums receding?", "Should I consider Invisalign?" No question is too small.
Do not have insurance? That is okay. Our Signature Visit New Patient Special is specifically designed for patients without insurance: a comprehensive exam, professional cleaning, digital X-rays, and panoramic X-ray for $149.
Preparing the Night Before
There is no special preparation required for a dental exam, but a few small steps can make the appointment more efficient:
- Brush and floss normally before your visit. There is no need to do anything extra--we want to see your mouth in its typical state so we can accurately assess your oral health.
- If you take medications for anxiety, confirm with your prescribing physician whether it is appropriate to take your regular dose before a dental appointment.
- Eat a normal meal before your visit. Low blood sugar can increase anxiety and make anesthesia feel more uncomfortable if treatment is needed.
- If you have dental anxiety, consider arriving 5 minutes early to settle in rather than rushing from the parking lot.
What Happens at Your First Appointment
Your first visit to Innova Smiles is designed as a thorough "get to know you" appointment--not a rushed assembly line. Here is what you can expect, step by step:
1. Warm Welcome and Check-In (5 to 10 minutes)
You will be greeted by our front desk team, complete any remaining paperwork, and get a tour of the office if you would like. We want you to feel comfortable in our space before anything clinical begins. Our office at 340 Maple St, Suite 100, Marlborough, MA 01752 is designed with patient comfort in mind--private treatment rooms, natural light, and a calm atmosphere that feels nothing like the sterile, institutional dental offices of the past.
2. Health History Review (10 minutes)
One of our team members will review your health history, medications, allergies, and any specific concerns or goals you have for your smile. We take time here because your overall health directly affects your dental health. Conditions like diabetes, heart disease, autoimmune disorders, pregnancy, and even acid reflux can change how we approach your care.
According to the Journal of Periodontology (2018), patients with uncontrolled Type 2 diabetes have a threefold increased risk of periodontal disease progression compared to non-diabetic patients. Similarly, medications like antihypertensives, antihistamines, and antidepressants can cause dry mouth (xerostomia), which accelerates tooth decay. Understanding your full medical picture allows Dr. Fatima to tailor your treatment plan appropriately.
3. Digital X-Rays (10 to 15 minutes)
We will take digital X-rays to get a complete picture of what is happening below the gumline--areas that are invisible during a visual exam alone. According to the ADA, digital X-rays are a safe and essential diagnostic tool for comprehensive dental care. Our digital X-ray system uses phosphor plate sensors that deliver 80 percent less radiation than traditional film X-rays. To put this in perspective, a full set of dental X-rays exposes you to roughly the same amount of radiation as a short airplane flight.
If you are pregnant, we will postpone non-emergency X-rays and adjust your care plan accordingly, following the ADA and American College of Obstetricians and Gynecologists guidelines.
For most new patients, we also take a panoramic X-ray (panorex)--a full-mouth image that shows your jaw joints (TMJ), sinuses, wisdom teeth, and all 28 to 32 teeth in a single image. This gives us the complete picture we need to assess your overall dental health, screen for pathology (cysts, tumors, bone loss), and plan any future treatment.
In complex cases involving implant planning, impacted wisdom teeth, or suspected jaw pathology, we may also take a 5D CBCT scan--a three-dimensional X-ray that provides millimeter-level detail of bone structure, nerve canals, and tooth roots. This technology eliminates guesswork and ensures precise diagnosis and treatment planning.
4. Periodontal (Gum) Evaluation (5 to 10 minutes)
Your hygienist will check the depth of spaces (pockets) between your teeth and gums using a small measuring tool called a periodontal probe. Healthy pocket depths are 1 to 3 millimeters. Depths of 4 millimeters or more may indicate early gum disease (gingivitis) or more advanced periodontal disease.
The CDC reports that 47.2 percent of American adults aged 30 and older show signs of periodontal disease--nearly half the population. In adults over 65, prevalence jumps to 70.1 percent. These pocket depth measurements, combined with X-ray evaluation of bone levels, tell us exactly where you stand and what level of cleaning you need.
Your hygienist will also assess gum recession, bleeding on probing (a sign of active inflammation), tooth mobility, and furcation involvement (bone loss between the roots of multi-rooted teeth). All of these measurements are recorded in your chart and compared at future visits to track trends over time.
5. Professional Cleaning (30 to 45 minutes)
Your hygienist will remove plaque (soft bacterial film) and tartar/calculus (hardened deposits that brushing cannot remove) from above and below the gumline. After scaling, your teeth are polished with a fine-grit prophylaxis paste to smooth enamel surfaces and remove superficial stains. A thorough flossing follows.
If significant tartar buildup is present--common in patients who have not had a cleaning in several years--a deeper cleaning called scaling and root planing (SRP) may be recommended at a separate appointment. SRP is performed under local anesthesia, typically one or two quadrants at a time, and involves cleaning below the gumline and smoothing root surfaces to promote gum reattachment. A meta-analysis in the Journal of Clinical Periodontology (2015) found that SRP reduced probing depths by an average of 1.29 mm in pockets initially measuring 5 mm or deeper.
6. Comprehensive Exam with Dr. Fatima (15 to 20 minutes)
Dr. Fatima will review your X-rays and exam findings with you on-screen, check every tooth for decay, cracks, and wear patterns, assess your bite and jaw alignment, screen for oral cancer (visual and tactile examination of the tongue, floor of the mouth, palate, and throat), and examine your jaw joints (TMJ) for clicking, popping, or pain.
The oral cancer screening is particularly important. The Oral Cancer Foundation reports that approximately 54,000 Americans are diagnosed with oral or oropharyngeal cancer each year, and early detection improves the five-year survival rate from 50 percent (late-stage) to over 80 percent (early-stage). This screening takes less than two minutes and is included in every exam at no additional cost.
Dr. Fatima will take the time to explain exactly what she finds--no dental jargon, no rush. She will show you X-ray images and intraoral photos on our chairside monitors so you can see what she sees.
7. Treatment Plan Discussion (10 to 15 minutes)
If any concerns are found, Dr. Fatima will explain them clearly, discuss your options (including doing nothing, when appropriate), and outline a treatment plan that fits your goals and budget. She will prioritize urgency: teeth that are actively infected or at risk of fracture come first, followed by preventive and maintenance care, followed by elective cosmetic improvements.
Our treatment coordinator will then review the financial side: insurance coverage, estimated out-of-pocket cost, and financing options. We never pressure patients into same-day treatment decisions. You will leave with a written treatment plan and a clear understanding of timelines, costs, and priorities.
Total Time
Plan for approximately 60 to 90 minutes for a thorough new patient visit. We prefer to do this right the first time rather than rush through an initial exam and miss something important. Patients from Grafton and Berlin tell us that the thoroughness of this first visit is what convinced them to make Innova Smiles their permanent dental home.

If You Have Not Been to the Dentist in Years
No judgment. We hear this often--from busy MetroWest professionals, parents who prioritized their kids' appointments over their own, and patients who developed dental anxiety after a bad experience years ago. Life happens. Here is what to know:
- You will not be lectured. Our team is compassionate, not judgmental. Guilt has never motivated anyone to take better care of their teeth, and we do not practice that way.
- We will start where you are. Not where we wish you had been. Your treatment plan begins from your current reality, not from an idealized version of what could have been.
- Problems are usually more manageable than feared. In Dr. Fatima's experience, the vast majority of patients who return after a multi-year gap discover that the situation is better than they imagined. Yes, there may be a few cavities or early gum disease, but catastrophic scenarios are the exception, not the rule.
- We can address dental anxiety head-on. Nitrous oxide (laughing gas) provides mild sedation and anxiety relief within minutes and wears off completely within five minutes of removing the mask. Our DentalVibe vibration anesthesia device makes injections virtually painless using the Gate Control Theory of Pain. For patients with moderate to severe anxiety, oral sedation options are also available.
A 2019 study in the British Dental Journal found that the most common reasons adults avoid dental visits are fear of pain (40 percent), cost concerns (35 percent), and embarrassment about the current state of their teeth (22 percent). We have designed our practice to address all three: sedation options for pain, transparent pricing with membership and financing plans for cost, and a strictly no-judgment clinical culture for embarrassment.
If you are nervous, just tell us when you call. We will take extra care to go slowly, explain each step before performing it, and check in frequently. You are always in control--raise your hand at any time and we stop immediately.
What to Expect After Your First Visit
After your initial appointment, here is what happens next:
- If no treatment is needed: We will schedule your next cleaning and exam in six months. You will receive a reminder by text, email, or phone (your choice) two weeks before the appointment.
- If treatment is recommended: You will leave with a written treatment plan prioritized by urgency. Our treatment coordinator will call to schedule follow-up appointments at times that work for your schedule. There is no expiration date on your treatment plan, but we do recommend addressing urgent items within 30 to 60 days to prevent progression.
- Insurance claims: We file all insurance claims electronically on your behalf. Most claims are processed within 2 to 4 weeks. If there is any discrepancy between the estimated and actual insurance payment, we will contact you before billing the difference.
- Records transfer: If you are switching from another dentist, we can request your records (X-rays, treatment history, periodontal charting) from your previous provider with your written authorization. This gives us a longitudinal view of your dental health.
Insurance and Payment
We accept most major PPO dental insurance plans, including:
- Delta Dental (Premier and PPO)
- Blue Cross Blue Shield
- Aetna, Cigna, Guardian
- United Healthcare, MetLife
- Humana, Ameritas, and more
We are in-network with most major PPO plans in Massachusetts, which means your out-of-pocket costs are based on negotiated rates rather than full fee. Our treatment coordinators verify your specific benefits before your appointment so you know exactly what to expect.
No insurance? We offer:
- The $149 Signature Visit New Patient Special
- 0 percent interest financing through CareCredit and Cherry (6 to 24-month terms available)
- Our in-house Innova Membership Plan — a flat annual fee covering two cleanings, exams, all diagnostic X-rays, and meaningful discounts on every procedure. No deductibles, no annual maximums, no waiting periods, no claim forms.
For patients weighing their options, our detailed guide on maximizing dental insurance benefits explains how to get the most from your coverage throughout the year.
Choosing the Right Dentist: What to Look For
If you are comparing dental practices in MetroWest, here are the factors that matter most for your first dental visit:
- Credentials and continuing education: Dr. Fatima holds Fellowships in the International Congress of Oral Implantologists (FICOI) and the American Academy of Implant Prosthodontics (FAAIP), reflecting hundreds of hours of advanced training beyond dental school. Ask any dentist you are considering about their post-graduate certifications.
- Technology: Digital X-rays, intraoral scanners, CBCT imaging, and CEREC same-day crowns are no longer luxuries--they are the standard of care at modern practices. Offices still using film X-rays and putty impressions are working with 1990s technology.
- Private practice vs. corporate dental chain: At a private practice like Innova Smiles, you see the same dentist every visit, treatment decisions are made by the dentist (not a corporate formula), and appointment times are not artificially compressed to maximize throughput. For a deeper comparison, read our guide on DSO vs. private practice dentistry.
- Reviews and reputation: Check Google reviews, ask friends and family in Hudson, Northborough, or Southborough for recommendations, and pay attention to how the office handles negative reviews (responsiveness and professionalism matter).
- Comfort amenities: Nitrous oxide, DentalVibe, oral sedation, blankets, headphones, and a calm environment are signals that a practice takes patient comfort seriously.

Frequently Asked Questions
Do I need to fill out forms before my appointment? We offer online forms you can complete at home for a faster check-in. If you prefer, you can also complete them in the office--just arrive 10 to 15 minutes early. The forms cover health history, medications, dental history, insurance information, and consent for treatment.
What if I am anxious about the dentist? Tell us when you call or when you arrive. We offer nitrous oxide (laughing gas), DentalVibe vibration anesthesia, and oral sedation for moderate to severe anxiety. You can also bring headphones to listen to music or a podcast during treatment. A study in Anesthesia Progress (2019) found that 88 percent of dental-phobic patients who received nitrous oxide sedation rated their anxiety as "manageable" or "minimal" during treatment.
Can I bring my child to my appointment? Yes--we are a family-friendly practice and welcome children. We offer pediatric dental care for patients of all ages. If you would like to schedule your child's appointment at the same time, just let us know when you book and we will coordinate.
What if I need more work done than a cleaning? We will explain everything we find and give you time to decide. We never pressure same-day treatment. If cost is a concern, we will build a treatment plan that prioritizes the most urgent needs and spreads less urgent work across multiple visits. Financing through CareCredit and Cherry is available for larger treatment plans.
Is the $149 special available if I have insurance? The special is designed for patients without insurance coverage for preventive care. If you have insurance that covers cleanings and exams, we will bill your insurance instead (which is typically better for you, since most PPO plans cover preventive visits at 100 percent with no copay).
How often should I visit the dentist? The ADA recommends at least two visits per year for most adults--one cleaning and exam every six months. Patients with active gum disease, high cavity risk, or complex restorations may benefit from three to four visits per year. Dr. Fatima will recommend a recall schedule tailored to your specific needs.
What is the difference between a cleaning and a deep cleaning? A standard prophylaxis (cleaning) removes plaque and tartar from above the gumline and is appropriate for patients with healthy gums or mild gingivitis. A deep cleaning (scaling and root planing) is a therapeutic procedure for patients with periodontal disease--it removes calculus from below the gumline and smooths root surfaces to promote healing. Deep cleanings are performed under local anesthesia, typically over two visits (one side of the mouth per visit).
