A dental exam is a check of the teeth, gums and soft tissues, including an oral cancer screening, with X-rays when needed. Cleaning is a separate service at the same visit; preventive care adds fluoride varnish, sealants on children’s back teeth and a home-care plan. At Innova Smiles in Marlborough, MA, Dr. Ambereen Fatima, DDS and a licensed hygienist see children from the first tooth, teens, adults and seniors.

In short
- Children from the first tooth, teens, adults and seniors are seen in one office, and you see the same dentist, Dr. Fatima, at every visit.
- A wellness visit takes about 60 minutes: health history, exam, periodontal charting, oral cancer screening, and digital X-rays when indicated. Cleaning is a separate service done the same visit when appropriate.
- Most patients are recommended every six months; patients with a history of gum disease are usually seen every three to four months for periodontal maintenance.
- Fluoride varnish is applied at preventive visits when recommended, and sealants on children’s back teeth mean up to 80% fewer cavities in those teeth for two years (CDC). At home, the ADA advises two minutes of brushing twice a day with fluoride toothpaste and cleaning between the teeth once a day.
- The new-patient visit (exam, cleaning and X-rays) is $0 when a cleaning benefit is available this year, in-network or out, or $149 for uninsured new patients. We accept all PPO plans; benefits are verified once you book and return your forms, and you see a written estimate before any treatment.
Care Selected for Your Needs
Your exam assesses your oral health. Imaging and preventive treatments are selected according to your needs; X-rays and cleaning are separate items rather than automatic parts of every exam.
See What We See
We use intraoral cameras to put a close-up of your own mouth on the screen. You get to see a crack or a spot of decay yourself, before it starts to hurt, and decide what to do with that information.
Early Detection
High-tech scanners catch issues while they are still small, saving you from expensive root canals or crowns later.
Patient Education
We put the images on a big screen so you can ask questions and feel confident about any treatment decisions.

Transparency in every visit.
The Gentle Checkup Journey
No clock-watching, no upsells. Here is how a visit at Innova Smiles actually runs.
Guided Health History
We review your medical history and any specific concerns you have. This baseline helps us personalize every cleaning and exam to your unique needs.
Digital Diagnostics
When indicated, low-radiation X-rays and intraoral photos give us (and you) a clear view of hard-to-see areas between and inside teeth. X-rays are a separate item.
Separate Cleaning, When Appropriate
Cleaning is a separate service. When appropriate, our hygienist can provide cleaning and polish during the same visit; you may also choose an exam without cleaning.
Personalized Wellness Plan
Dr. Fatima reviews findings with you and outlines a path forward, including options for whitening or aligners.
A Better Exam, In Practice
- Comprehensive Exam: We assess your teeth, gums, bite, and existing dental work to catch issues before they escalate.
- Digital Imaging: X-rays and photos are selected as needed for treatment planning. X-rays are a separate item from the exam.
- Separate Cleaning: A licensed hygienist can provide cleaning when appropriate. This is a separate service, not an automatic part of the exam.
- Personalized Roadmap: Leave with clear next steps—whether it's preventive add-ons, cosmetic goals, or restorative follow-up.
- Catch cavities and cracks before they require extensive care.
- Track gum health with measurements and targeted coaching.
- Keep gums healthy, which research associates with heart health and diabetes control (see why it matters).
- Keep cosmetic and orthodontic results looking their best.

One Dentist for Every Age
Innova Smiles is a dental home for the whole household. Children are seen from the first tooth, with a first visit by the first birthday as the AAPD and ADA advise, and move on to sealants and fluoride as the back teeth come in. Teens get hygiene coaching and an orthodontic evaluation, with Spark clear aligners when alignment is the goal. Adults come for cleanings, fillings, crowns and cosmetic work; seniors for gum care, crowns and bridges, dentures and implant-supported options.
You see Dr. Fatima at every visit, not a rotating roster, so your family’s history and preferences carry from one appointment to the next, and the team speaks English, Spanish and Portuguese. Read more about children’s dentistry, orthodontics for teens and adults, and restorative care for seniors.

Fluoride Varnish & Dental Sealants
Two preventive treatments sit alongside the exam and cleaning. Both are selected according to your needs and, like X-rays, are separate items rather than automatic parts of every visit.
Fluoride varnish
Fluoride remineralizes weakened enamel and makes it more resistant to the acid attacks that cause cavities. Toothpaste and drinking water give baseline protection; a professional-strength varnish delivers a concentrated dose directly to the teeth. When recommended, it is painted on at the end of the cleaning, adheres to the enamel and releases fluoride over several hours, with no rinse-and-spit.
It is most often recommended for children and teens, for dry mouth caused by medications or medical conditions, for receding gums that expose root surfaces, and around crowns or braces where plaque collects.
Sealants for children’s molars
Sealants are thin coatings applied to the chewing surfaces of back teeth, the pits and grooves where most childhood cavities develop. They go on in minutes with no drilling, and sealed back teeth have up to 80% fewer cavities for two years (CDC). We check them at regular cleanings and can reapply them if needed.
Most PPO plans cover fluoride and sealants for children; benefits are verified once you book and return your forms. Read more on our children’s dentistry page.
Preventive Care for Every Stage of Life
Regular dental exams aren’t just about your teeth, they’re an investment in your family’s total health.

- Visit every six months to prevent decay and gum disease before symptoms appear.
- Patients with a gum disease history are usually seen every three to four months for periodontal maintenance.
- Schedule sooner if you're pregnant, managing chronic conditions, or notice bleeding gums.
- Ask about custom night guards or fluoride boosts if you grind, sip coffee, or experience sensitivity.
Home Care Between Visits
Most prevention happens in the six months between cleanings. These are the home-care basics Dr. Fatima recommends to every patient at Innova Smiles in Marlborough, MA, following American Dental Association guidance.
Brushing and Cleaning Between Teeth
Brush for two full minutes twice a day with fluoride toothpaste and a soft-bristled brush, held at a 45-degree angle to the gumline so the bristles reach just under the gum margin, and replace the brush or brush head every three to four months. Pressing hard does not clean better; it wears enamel and pushes the gums back.
Clean between the teeth once a day with floss, interdental brushes or a water flosser; brushing alone misses the surfaces between teeth. Plaque left there can begin to harden into tartar within a day or two (American Dental Association), and once it has, only a professional cleaning removes it.
Food, Drink and Timing
Each sugar exposure starts an acid attack on enamel that lasts about 20 minutes (ADA), so sticky candy, soda, and sipping sweet or acidic drinks over hours do more harm than the same amount taken at one sitting. Water is the drink that does the least damage.
After acidic food or drink, rinse with plain water and wait about an hour before brushing, because softened enamel abrades. A dry mouth from medications raises cavity risk, so mention it at your visit; prescription fluoride and more frequent cleanings are options.
Exam & Cleaning Costs
Preventive care is the most cost-effective way to maintain your oral health. Most dental insurance plans cover exams and cleanings at 80–100%, many patients pay little to nothing out of pocket. Ask about our new patient special for uninsured patients.

Why Regular Exams and Cleanings Matter
Protecting your teeth starts with regular exams and cleanings. Preventing dental disease costs less and hurts less than treating it after painful symptoms arise.
A routine dental exam at our Marlborough clinic goes well beyond a quick glance for cavities. We use low-radiation digital X-rays to see beneath the gumline and inside the teeth.
Even if you brush and floss well, hardened tartar (calculus) builds up in spots a toothbrush can't reach. At your cleaning, our hygienist uses ultrasonic instruments to remove that buildup, the part of the visit a home routine can't replace.
The Mouth and the Rest of the Body
Research has reported associations between chronic gum infection and conditions elsewhere in the body, including cardiovascular disease, Type 2 diabetes and adverse pregnancy outcomes; the bacteria behind periodontal disease can enter the bloodstream. An association is not proof that treating one prevents the other, but keeping the mouth free of infection is one practical part of looking after your overall health.
Dental disease is also cumulative. A small cavity left alone becomes a large one; a cavity that reaches the pulp needs a root canal; a tooth weakened by repeated treatment may eventually need a crown, bridge or implant, and each step costs more time and money than the one before it. Many serious conditions, including oral cancer, bone loss and cracked teeth, produce few symptoms until they are advanced, so regular exams give the team the chance to find them earlier, often while conservative treatment is still an option.

What Happens During Your Comprehensive Dental Exam?
A comprehensive dental exam at Innova Smiles is a full assessment of your oral health. Cleaning is a separate service, and you may choose an exam without cleaning. We begin with digital X-rays when indicated to check for decay between teeth, bone levels around roots, and abnormalities beneath the gum line. Dr. Fatima then performs a visual examination of all teeth, gums, and soft tissues, including an oral cancer screening of the tongue, cheeks, and throat, to look for changes that may need follow-up.
A periodontal (gum) assessment measures the depth of the space between your teeth and gums at six points per tooth. Depths above 3mm indicate inflammation or disease that requires more than standard cleaning. We also evaluate your bite, jaw joint function, and the condition of any existing restorations. All findings are documented in your chart so we can track changes at every visit and intervene early when needed.
Professional Cleaning vs. Scaling & Root Planing: What’s the Difference?
A standard prophylaxis (professional cleaning) removes plaque and tartar from above the gum line and lightly below it, followed by polishing. This is appropriate for patients with healthy gums or mild gingivitis. Scaling and root planing (SRP), or deep cleaning, goes significantly further, removing calculus from deep below the gum line (up to the root surface) and smoothing root surfaces to discourage bacterial reattachment. SRP is the clinically indicated treatment for periodontal disease.
The determination of which type of cleaning you need is based on your periodontal measurements, not the hygienist’s preference or your insurance category. Patients diagnosed with active gum disease are placed on a periodontal maintenance schedule, typically every 3–4 months instead of every 6, until their condition is stabilized. This is a clinical recommendation, not an upsell. Consistent periodontal maintenance is the evidence-based approach to managing gum disease and preventing tooth loss.

Dental Exam & Preventive Care FAQs
We recommend scheduling a professional dental exam and cleaning every six months. These biannual visits allow our Marlborough MA dentist team to catch early signs of decay, monitor your gum health, and perform routine oral cancer screenings before small issues become painful or expensive problems.
Most PPO dental insurance plans cover two preventive exams and cleanings per year at 100%, meaning zero out-of-pocket cost for you. If you do not have insurance, our dental office near Southborough and Westborough offers a $149 New Patient Special that includes a comprehensive exam, all necessary X-rays, and a professional cleaning. For ongoing savings, our In-House Membership Plan ($395/year for adults) includes all your routine preventive care — two cleanings, two exams, and all X-rays — plus a flat 20% off our standard fees on additional treatments. It is a savings plan, not insurance. We also accept CareCredit and Cherry financing for any remaining costs. Once you've booked your appointment and returned your forms, we'll verify your insurance benefits and call you with a breakdown within three business days.
We recommend dental sealants for children and teenagers as soon as their permanent back molars come in. A sealant is a thin protective coating painted into the chewing grooves of the tooth. It blocks cavity-causing bacteria and makes the surface easier to keep clean.
For most healthy patients, we recommend updating routine checkup X-rays once a year to detect cavities hidden between teeth. If you have advanced gum disease or are undergoing extensive restorative work, we may need to take completely safe, low-radiation digital X-rays slightly more frequently.
The ideal time for a child's first dental visit is within six months of their first tooth erupting, or by their first birthday. This early introductory visit helps them build comfort in the dental chair while allowing us to monitor early jaw development.
A regular cleaning removes tartar above the gumline for healthy patients. A deep cleaning, or scaling and root planing, is required when active gum disease is present. We must clean deep beneath the gums and carefully smooth the tooth roots to comprehensively eliminate harmful bacteria.
Yes, professional fluoride treatments provide profound benefits. Fluoride acts as a shield, directly remineralizing weak enamel and making it much more resistant to acid attacks. A quick, comfortable varnish application significantly lowers your child's overall risk of developing painful cavities.
Bleeding gums are often the first sign of gingivitis (early-stage gum disease) caused by plaque buildup along the gumline. Healthy gums should not bleed during normal oral hygiene. If you notice bleeding, it's a signal to book a cleaning. We can gently remove the tartar that brushing alone can't reach, stopping the inflammation before it leads to bone loss or more serious health issues.
The most effective cavity prevention combines good home care with regular professional care: brush twice a day with fluoride toothpaste, clean between your teeth daily with floss or interdental brushes, limit sugary and acidic snacks and drinks, and drink fluoridated water. Professional cleanings and exams every six months catch decay early, and treatments like fluoride and dental sealants add extra protection, especially for children. At Innova Smiles in Marlborough, MA, we tailor a prevention plan to your risk factors and use digital X-rays to spot problems before they grow.
Preventive care keeps small problems from becoming painful, expensive ones. Regular exams and cleanings remove the hardened plaque that brushing can't reach, catch cavities and gum disease early, and include an oral cancer screening. Because oral health is linked to overall health, preventing gum disease may also support your general wellness. For most people, a checkup every six months is the simplest way to protect both your smile and your budget. At Innova Smiles in Marlborough, MA, we focus on prevention first for patients of all ages.
Mild sensitivity for a day or two after a cleaning is common and usually harmless. It can happen when tartar that had built up along the gumline is removed, briefly exposing the tooth, or if your gums were inflamed beforehand. Using a toothpaste made for sensitive teeth, avoiding very hot or cold foods for a day, and brushing gently usually helps. If the sensitivity is severe, lasts more than a few days, or is focused on one tooth with pain when biting, call us so we can check it.

Our team is experienced in caring for patients from childhood through adulthood, from a child's first visit (recommended by age 1) to personalized care for seniors.
